It's about Roger Waters' daughter. She was on vacation when Roger had his birthday party with the family. That really upset him.

They say that is why he fired her from the band.

The story acctually comes from when roger stubbed his toe quite badly and rushed to the doctor in town. When he arrived in pain a sign on the door said "Out to Lunch". He cried in anguish "I WISH YOU WERE HERE!" and the song came from there.
